Abstract

This Paper Discusses Machine Controls That Fail, And Typical Causes For The Failures. Some Are The Predictable Result Of Fundamental Control Algorithm Deficiencies, And Others Are The Result Of Neglecting Physical Realities In The Industrial Environment Where The Controls Must Operate Reliably And Safely. Also Included Are Explanations Of Several Phenomenon And Explanation Of Failure Modes, As An Aid In Identifying The Various Possible Failure Modes That May Be Involved.
